Comes exactly as shown. Works great. There is no way to collect information about usage hours, as the site for putting in the codes was shut down. HOWEVER, the screen still has the protector on it, which is a good sign. No, unfortunately no case. If you are interested in hoses, I do have some basic no name hoses and resmed hoses, I will create a custom listing upon request, as Im going to include extras with the hoses to sweeten the deal 🤑
